BYU personnel have until March 15 to use 2005 FSA funds

FSA claims are due April 30

It’s time to for full-time BYU personnel to “wrap-up” their 2005 Flexible Spending Accounts.

University personnel who established 2005 Flexible Spending Accounts (FSA) have until March 15 to use the money for eligible purchases and services. Claims for 2005 must be submitted by April 30. Any money left in the account after April 30 will be forfeited.

FSA claims for (1) eligible medical and dental expenses processed by DMBA and (2) prescriptions filled by Medco contracted pharmacies can be processed on-line. This is done by going to the DMBA web site, http://www.dmba.com and clicking on “Submit FSA Claims.”

FSA claims for eyeglasses, contacts, over-the-counter medicines, daycare, etc., still need to be mailed to DMBA along with receipts. Employees enrolled with Altius must file paper claim forms for all services.

Claim forms are available on the DMBA web site or in the Benefits Office, D-240 ASB.

Please contact Benefits Services at ext. 2-4716 or at benefits@byu.edu if you have any questions.
